Certificate Advising
Here are the forms and information you will need as you progress through a certificate.

Graduation Information
In order to graduate from the MSP, you must enroll in and successfully complete:

Graduation Application Packet (Documents)
Graduation Application packets are due approximately one week before the first day of the semester you plan to take Portfolio Development. This packet must include:
Instructions
Use the links above to download the necessary documents and print them. Make sure that Portfolio Development is the ONLY class on the registration form that you submit with your application. On the graduation application, fill out the tracking sheet, including courses you have completed, and any courses in which you are currently enrolled. Be sure to indicate for each course the semester and year that you did complete it or will complete it. If there are courses that you want to use as substitutes, check with MSP staff first and then submit a Request for Course Waiver/Substitution form with your packet for each substitution. If you took classes that are older requirements not listed on your tracking sheet, please write them in the "Substitutions" spaces, next to the name of the course that replaced it, or its nearest equivalent. Deadlines
The deadline to submit the graduation application is one month before the start of your final semester.
You may submit your packet to the front desk at the downtown campus, addressed to Carmen, or you may mail it to:

Grad Page Guidelines
The MSP would like to show your work to the world. You may submit the following items to have a personal "Grad Page" created for you:
- One optimized, Web-ready photo of you (maximum width 400 pixels)
- Your e-mail address, if you would like it to be on your grad page
- Up to three optimized, Web-ready images of your work (maximum width 400 pixels)
- Your name as you would like it to appear on the grad page
- A short bio (one paragraph)
- Your certificate track
- The URL for your portfolio site
- A short description of your portfolio
- The tools (hardware & software) you used to create your portfolio
This content must be submitted, on a ZIP DISK or CD ONLY, to the information desk, addressed to Carmen Gonzalez. The deadline to submit your grad page materials will be three weeks before your graduation ceremony date. Your grad page materials will be returned to you at the graduation ceremony.
Unofficial Transcripts
Unofficial Transcripts can be downloaded and printed from the main SF State Web site. Please go to https://www.sfsu.edu/online/login.htm and enter your Personal Access Code (this will be the same code you use to register for classes using TouchTone, usually the DAY and YEAR of your birth as a four-digit number) and student ID. Then click on the button "Get my information choices." This will bring up a list of information that you can download. Click on the button next to "Unofficial Transcripts."
